Ruslan Kuprieiev 5d381da5d9 ssh: change order of user configuration 
Unfortunately this breaks backward compatibility, but we have to do it,
because current order is totally wrong. Configuring user with `dvc
remote modify myssh user` is an explicit action and should have higher
priority than implicit url. This is also important, since in the next
patch in this PR we will introduce support for `port` configuration,
that should also follow the same logic.

Ruslan Kuprieiev 7cd24e0c1f RFC: dependencies/outputs: unify classes 
There is a lot of duplication in current Output/Dependency classes, that was
left "just in case" while developing different schemes. Since our code base is
now getting more and more stable and we now know what we need to add support
for any new dep/out type in the future, we can now safely unify most of the
code.

Ruslan Kuprieiev d5dd0fd804 test: don't use sleep 
Since we now use file size to help detect file changes on filesystems
with bad mtime resolution, we no longer need these explicit sleep()
calls in our tests for mac os.
